C is where the H&C/Circle goes over the road between Wood Lane and Latimer Road
Don’t know the name of the road but it links the A40 and the Shepherds Bush Roundabout.
All the pictures look like they are on the H&C/Circle

A is the approach to Westbourne Park from the west. Based on what @aetearlscourt and John Tuthill have said, this implies that the locations are heading west along the Hammersmith branch with B being between Ladbroke Grove and Latimer Road and the 5th in the set being between Shepherd's Bush Market and Goldhawk Road.
