I know this is unlikely but I don't think it outside the realms of possibility.

I don't think that's a major problem - the original DLR cars didn't have end doors anyway. On the NR network much of the stock doesn't have end doors - for example a Class 170. If a train were to derail, and fall over sideways in such a way that the cars stayed together and the corridor connection didn't break, then you would simply smash the front windscreen.
