"EAST HAM STATION: No step free access between the ticket hall and the eastbound and westbound platforms due to non availability of staff."
Why does a lack of staff mean that there is no step-free access? I've never alighted at East Ham, but I doubt that there are lift attendants there?

I think that some lifts need to have staff assistance to operate for safety reasons.
they need to unlock it with the key, because hydraulic lifts can overheat.

It means there was not enough trained staff to assist people if the lift broke down.
As to faults, low oil pressure, oil overheating [happened to my one earlier! ;D] Car locks broken etc.

With East Ham being my home station, I'd say I more or less agree with Rob. When the lifts were first installed, a lot of the local kids used to abuse them quite a lot, and at one point they took such a bashing that the lifts were out of service for quite a while.
It depends what time of day it is as well. During the peak, the lifts are left to run by themselves, but towards the evening and late at night (when there's practically nobody on the gateline), if you press the lift's call button, an alarm bell starts ringing, and this prompts a member of staff to come out and get the lift running.
